- Chondrocyte Viability in Fresh and Frozen Large Human Osteochondral Allografts: Effect of Cryoprotective AgentsPublication . Judas, F; Rosa, S; Teixeira, L; Lopes, C; Mendes, AFChondrocyte survival is a major goal for the effective storage and clinical performance of human osteochondral allografts. The majority of animal and human cryopreservation studies conducted so far have been performed in small osteochondral cylinders. Using human tibial plateaus as a model for large osteochondral pieces, this work sought to evaluate the cryoprotective efficiency of glycerol and dimethylsulfoxide (DMSO), and to identify cryopreservation conditions suitable for use in tissue banks. Human tibial plateaus harvested from 7 cadaveric tissue donors were incubated in the presence or absence of cryoprotective agents (CPA): 10% or 15% glycerol and 10% DMSO in a Ham F-12 nutrient mixture. Chondrocyte viability was assessed immediately after thawing, using the MTT reduction assay and a fluorescence microscopic method. The tibial plateaus frozen in the absence of CPA showed a significant decrease in chondrocyte viability. The use of CPA significantly increased chondrocyte viability compared with cartilage frozen without CPA (nearly 50% versus 80% living chondrocytes with 10% glycerol versus 10% DMSO, respectively) relative to that in fresh cartilage. In this regard, 10% DMSO was slightly more effective than either 10% or 15% glycerol, eliciting the recovery of approximately 15% relative to the living chondrocyte content in fresh cartilage. In all conditions, fluorescence microscopic studies showed that surviving chondrocytes were restricted to the superficial cartilage layer. Human tibial plateaus seemed to be a good experimental model to establish cryopreservation methods applicable to large human osteochondral pieces in tissue banks.
- Cirurgia na Artrite Idiopática Juvenil: Caso ClínicoPublication . Judas, F; Teixeira, L; Costa, P; Dias, AApresenta-se o caso clínico referente a uma doente com 17 anos de idade, portadora de artrite idiopática juvenil poliarticular, que apresentava uma anquilose da anca esquerda com um flexo de 35º e uma rigidez em extensão do joelho esquerdo de 20º. Por mecanismo compensatório, instalou-se progressivamento uma hiperlordose lombar, uma bacia oblíqua e um equinismo do pé homolateral. Apesar da terapêutica farmacológica e da instituição de um programa intenso de fisioterapia, verificou-se um agravamento progressivo das deformidades dolorosas da anca e do joelho, condicionando uma marcha só possível com o auxílio de canadianas. Procedeu-se, num primeiro tempo cirúrgico, à implantação de uma prótese total autobloqueante cimentada na anca esquerda e, passados 2 meses, ao desbridamento intra-articular do joelho por via artroscópica. Presentemente a doente deambula sem canadianas, apresenta uma anca e joelho indolores com mobilidades próximas da normalidade, manifestando um grau elevado de satisfação face ao resultado dos procedimentos cirúrgicos. Como conclusão, importa dizer que, por um lado, apesar da idade jovem da doente só uma artroplastia da anca conduz a uma solução satisfatória e, por outro lado, o resultado alcançado pelo desbridamento do joelho por via artroscópica permite um ganho de tempo precioso, em relação à realização de uma eventual artroplastia total do joelho.
- Coimbra University Hospitals' bone and tissue bank: twenty-two years of experiencePublication . Judas, F; Teixeira, L; Proença, AWe report the microbiological contamination rate of sterilely procured 3953 tissue allografts obtained during 22 years of activity for musculoskeletal reconstruction from 1982 to 2003. From 1987 to 2000, allograft retrievals were performed in 191 cadaveric donors and in 323 living donors. In the former group 30 retrievals (15.7%) were excluded based on laboratory criteria. Among living donors 108 femoral heads (33.4%) were also excluded by the same criteria. The microbiological contamination rate of sterilely procured allografts in the operating room was 8.3% for cadaveric donors and 18.2% for living donors. A questionable positive serology for HIV antigen was registered in two non-heart-beating donors. Hepatitis C virus antibodies were positive in two other non-heart-beating donors. Hepatitis B virus serological markers were positive or questionable in more than 11 non-heart-beating donors. In living donors 20 femoral heads were excluded (6.1%) due a positive or questionable hepatitis B virus serology. One femoral head donor showed a positive HTLV-I antibody and another one a positive syphilis serology. No positive serology cases for the HIV antibodies were found. No cases were registered of transmission of viral diseases from the donor to the recipient. Our extremely rigorous criteria led to the exclusion of a considerable number of both donors and allografts.

- Fast volumetric registration method for tumor follow-up in pulmonary CT examsPublication . Silva, JS; Cancela, J; Teixeira, LAn oncological patient may go through several tomographic acquisitions during a period of time, needing an appropriate registration. We propose an automatic volumetric intrapatient registration method for tumor follow-up in pulmonary CT exams. The performance of our method is evaluated and compared with other registration methods based on optimization techniques. We also compared the metrics behavior to inspect which metric is more sensitive to changes due to the presence of lung tumors.

In the past week vomiting and abdominal pain appeared suddenly. Anorexia developed and there was an average weight loss of about 5 Kg.
- Magnetic resonance imaging in the preoperative staging of endometrial carcinomaPublication . Cabrita, S; Rodrigues, H; Abreu, R; Martins, M; Teixeira, L; Marques, C; Mota, F; Oliveira, CFPURPOSE OF INVESTIGATION: Magnetic resonance imaging (MRI) has emerged as an important imaging modality in the evaluation of the extension of endometrial carcinoma which is essential in planning treatment and predicting prognosis. This study aimed to assess the value of MRI in the preoperative staging of endometrial carcinoma. METHODS: We included in this study 162 patients with a histological diagnosis of endometrial carcinoma who underwent MRI pelvic imaging and surgical staging. MRI images were compared with pathological findings to measure MRI's sensitivity, specificity, positive and negative predictive values and diagnostic accuracy in what concerns myometrial, cervical and lymph node invasion. RESULTS: MRI differentiation of deep myometrial invasion from superficial disease agreed with pathological findings in 77% of cases, with a sensitivity of 83%, a specificity of 72% and a diagnostic accuracy of 77%. Concerning cervical invasion, MRI had a sensitivity, specificity and diagnostic accuracy of 42%, 92%, 81% respectively. In assessing lymph node invasion, MRI presented a sensitivity of just 17%, a specificity of 99% and a diagnostic accuracy of 89%. CONCLUSION: Our study confirmed the high accuracy of MRI imaging in assessing myometrial and cervical invasion in endometrial carcinoma. When evaluating lymph node invasion, micrometastases are responsible for the low sensitivy of MRI.
