- A investigação nos programas de formaçãoPublication . Carvalho, A; Porto, APostgraduate medical training is intended to give competence in a specific field of medicine. Postgraduate programs should therefore be designed to offer the appropriate opportunities for the acquisition of specific knowledge and skills required for expertise in that particular medical or surgical specialty. The trained physician should not only be able to practice his specialty, but also have a deep understanding of the fundamental knowledge involved and convey this experience to others. Scientific curiosity, critical reasoning, persistence, and the ability to work in a team are important qualities that should be developed during post graduate training. Due to its requirements and methods, research clearly emerges as an important tool in promoting the timely achievement of these main objectives. All departments should be involved in basic or clinical research, and this work should be part of the postgraduate program. Finally, research activities should be appropriately valorized in the overall appreciation of the trainee's curriculum.
- Techniques of intestinal transplantation in ratPublication . Lopes, MF; Cartucho, DJ; Cabrita, AM; Patrício, JTwo surgical models of intestinal transplantation in the rat are described. One is the implantation of fetal and newborn intestine as free grafts into the omentum of adult recipients, the other the adult intestine transplantation as an accessory graft using vascular anastomoses. A hundred and sixteen small-bowel transplantations were done; 36 of which were fetal intestine (group I), 40 of newborn intestine (group II), and 40 of adult intestine (group III). In the fetal and newborn intestinal transplantation, we emphasize the practices that allowed us to avoid ischemic and traumatic injury to the graft. In the adult intestine transplantation with vascular anastomoses, we heighten the modifications in the surgical technique that made the operation easier and the strategies used to prevent hypothermia and hypovolemic shock. Once experienced with the two chosen surgical techniques, transplantation using an avascular segment became much easier and quicker than transplantation with vascular anastomoses

- Umbilical hernia of stomachPublication . Sampaio, R; Ferreira, MHerniation of the stomach through the umbilicus is exceedingly rare with only one case reported in the international literature in the past 40 years. One case of a reducible gastric umbilical hernia, not diagnosed by endoscopy, is reported. Diagnosis was made by double-contrast barium examination after 5 years of symptoms and ineffective treatment. Herniation of the stomach is difficult to diagnose by endoscopy and radiological studies can be more sensitive and allow a more specific diagnosis.
- Immune phenotype of chronic liver diseasePublication . Czaja, JA; Santos, R; Porto, A; Santrach, PJ; Moore, SBImmune disorders in chronic liver disease may reflect common host propensities or disease-specific factors. Our aim was to determine the principal bases for these expressions. Four hundred fifty-one patients with various chronic liver diseases were assessed prospectively for concurrent immune disorders. Individuals with immune diseases were more frequently women (73% vs 60%, P = 0.02) and they had HLA DR4 more often than counterparts with other HLA (46% vs 23%, P = 0.000008). The association between HLA DR4 and immune disease was apparent within individual liver diseases and within different categories of liver disease. Women with HLA DR4 had a higher frequency of immune disease than women without HLA DR4 (52% vs 22%, P < or = 0.000001), and they also had immune diseases more commonly than DR4-positive men (52% vs 31%, P = 0.03). DR4-positive men, however, had higher frequencies of immune disease than DR4-negative men, especially in the nonimmune types of liver disease (26% vs 4%, P = 0.002). We conclude that HLA DR4 and female gender constitute an immune phenotype that is an important basis for autoimmune expression in chronic liver disease.
- Brucella endocarditis of the aortic valvePublication . Leandro, J; Roberto, H; Antunes, MJBrucella endocarditis was diagnosed in two patients with acute renal failure. Both patients had major aortic insufficiency, congestive cardiac failure and clinical and laboratory signs of an active infection, although adequate antibacterial therapy had already been introduced. Replacement of the aortic valve, together with the aortic root in one of the cases, were carried out as emergency procedures, followed by antibacterial treatment with rifampicin, doxycycline and co-trimoxazole. Both patients left the hospital cured and are well 2.5 and 2 years after the surgery, respectively.
- Familial camptodactylyPublication . Brites, MM; Moreno, AJ; Salgado, M; Poiares-Baptista, ACamptodactyly is a permanent flexion deformity at the interphalangeal joints, usually at the proximal, interphalangeal joints. It is by far most commonly encountered in the little finger, but it can occur in any of the others, and may be associated with a deformity in the little finger or not. It is usually bilateral. It can also appear in the toes, the second toe being the most frequently affected. This deformity can appear in two different situations: it can occur in isolation, or can be a part of a malformative syndrome. It can also be sporadic or be transmitted as an autosomal dominant trait [1, 5, 7, 8]. We report a case of familial camptodactyly not associated with any other disorders: the differential diagnoses included scleroderma and Dupuytren's contracture.
