- Ectopic thyroid in the anterior mediastinumPublication . Guimarães, MJ; Valente, C; Santos, L; Baganha, MFEctopic thyroid is a rare condition, and its location in the anterior mediastinum is even rarer, there having been only 5 reported cases in the past 30 years. Here, we describe 2 clinical cases and present a review of the literature regarding the etiology, embryology and clinical manifestations of ectopic thyroid.
- Eficácia e tolerabilidade de próteses na via aéreaPublication . Valente, C; Catarino, A; Ferreira, J; Robalo-Cordeiro, CThe function of airway stents is to keep the tubular structures open and stable. Their insertion is essentially indicated for intrinsic obstruction or extrinsic compression of the airway, fistulae or tracheobronchomalacia. The aim of this study was to determine the tolera bility and efficacy of airway stents in situations in which their insertion was vital. A retrospective study of airway stent insertion with rigid bronchoscopy (23 patients) was carried out over a two year period (2006-2007) at the Diagnostic and Therapeutic Techniques Unit. We assessed indication, efficacy, tolerability, complications and exact insertion based on chest CT imaging. In all situations Dumon flexible silicon stents (Tracheobronxane were used, with the need for complementary techniques such as laser therapy and mechanical dilation having been previously evaluated by flexible bronchoscopy. The authors conclude that stent insertion has no complications and good tolerability in the majority of advanced stage oncological situations with indication for palliative management.
- Emerging and under-recognized Chagas cardiomyopathy in non-endemic countriesPublication . Cortez, J; Providência, R; Ramos, E; Valente, C; Seixas, J; Meruje, M; Leitão-Marques, A; Vieira, ADue to recent population emigration movements, an epidemic of Chagas disease is currently menacing most developed countries. The authors report the case of a 53-year-old Brazilian woman living in Europe for the last 10 years who developed heart failure symptoms, having a previous symptomatic sinus node disease with a pacemaker implant at age of 40 years. The diagnosis was based on serology and myocardial biopsy and the patient was treated with nifurtimox. The authors emphasize the need of a high level of suspicion in patients with suggestive epidemiology and the need of populational screening of specific high risk groups. New treatment options are also discussed.
- Global expression of Chagas´ disease - emerging opportunities and impact in PortugalPublication . Cortez, J; Ramos, E; Valente, C; Seixas, J; Vieira, AChagas disease, caused by the protozoan Trypanosoma cruzi is endemic in the countries of Central and South America. Despite vector control programs and other measures taken in the blood banks and maternity hospitals, it is estimated that there are about eight million people infected worldwide. Although traditionally associated with rural life and poverty, the current migration flows transform it into a global public health problem. In Portugal, this problem is poorly known, with an estimated underdiagnosis index that exceeds 99%. In European countries, besides imported cases, autochthonous infections arise through vertical transmission and blood/organ donation. The conventional serological tests for diagnosing Chagas disease and verifying its cure are indirect hemagglutination (IHA), indirect immunofluorescence (IFAT), and enzyme-linked immunoabsorbent assay (ELISA).The hypothesis of autoimmunity as a basic mechanism of this disease and the absence of early markers of cure are the causes of controversy regarding the specific treatment of this disease. The currently available drugs have adverse effects on a large number of patients and parasitological efficacy in chronic disease is suboptimal. The authors reinforce the need of a high level of suspicion in patients with suggestive epidemiology and the need of populational screening of specific high risk groups.

- Pneumonia eosinofílica crónica idiopática: A propósito de um caso clínicoPublication . Valente, C; André, S; Catarino, A; Loureiro, M; Baganha, MFThe eosinophilic pneumonias are a heterogeneous group of pulmonary disorders, which may compromise only the air ways, the pulmonary parenchyma, or both, characterised by alveolar eosinophils and infiltration of pulmonary tissue, with or without peripheral blood eosinophilia. Idiopathic Chronic Eosinophilic Pneumonia (ICEP), detailed description was by Carrington in 1969, is a rare eosinophilic lung disease, of unknown aetiology, characterised by peripheral blood eosinophilia, chest radiograph infiltrates and prompt response to corticosteroid therapy. ICEP most commonly affects women of middle age and usual symptoms are cough, dyspnea, fever and weight loss. The authors present a case of ICEP in a young woman, 21 years old, non-smoker and previously healthy.
